Home Infusion Toolkit v2.0

The Home Infusion Toolkit v 2.0 is designed to provide all hospitals that participate or want to participate in home infusion with an outline of the requirements of such a service and some sample documents to support it. This toolkit does not provide a homogenous home infusion program for the province of Ontario as the scope and structure of these programs varies from hospital to hospital.   These sample documents are to be used as a model only in creating your own organizational documents to support your hospital’s home infusion program. Those documents provided in WORD can be edited into hospital templates.   This toolkit will undergo a formal review in 3 years time. Any changes to the toolkit before that time will be communicated through our “What’s New”. We hope you find it helpful and always welcome any comments you may have.
