Notre parcours vers des stocks de cette nature a commenc\u00e9 en janvier 2022, lorsque la SCS a entrepris une mise en stock progressive de plaquettes \u00e0 teneur r\u00e9duite en agents pathog\u00e8nes. <\/figcaption><\/figure>\n\n<p class=\"wp-block-paragraph\">Pour rendre ses produits encore plus s\u00fbrs, la Soci\u00e9t\u00e9 canadienne du sang (SCS) vise \u00e0 stocker des composants et produits \u00e0 teneur r\u00e9duite en agents pathog\u00e8nes, dans la mesure du possible (figure 1). Notre parcours vers des stocks de cette nature a commenc\u00e9 en janvier 2022, lorsque la SCS a entrepris une mise en stock progressive de plaquettes \u00e0 teneur r\u00e9duite en agents pathog\u00e8nes. <\/p>\n\n<p class=\"wp-block-paragraph\" id=\"octaplasma\">En mars cette ann\u00e9e, la m\u00eame transition vers du plasma \u00e0 teneur r\u00e9duite en agents pathog\u00e8nes commencera par la mise en stock g\u00e9n\u00e9ralis\u00e9e d\u2019Octaplasma, un plasma trait\u00e9 par solvant\/d\u00e9tergent (S\/D). En outre, la mise en stock d\u2019un autre type de plasma \u00e0 teneur r\u00e9duite en agents pathog\u00e8nes est planifi\u00e9e pour la fin de 2024 ou 2025. Ce dernier composant sera collect\u00e9 et fabriqu\u00e9 par la SCS \u00e0 l\u2019aide d\u2019un autre type de technologie d\u2019inactivation des agents pathog\u00e8nes, puis offert en plus du plasma trait\u00e9 par S\/D. \u00c0 terme, on s\u2019attend \u00e0 ce qu\u2019environ 80 % du plasma utilis\u00e9 soit du plasma S\/D et que 20 % soit du plasma \u00e0 teneur r\u00e9duite en agents pathog\u00e8nes collect\u00e9 par la SCS. D\u2019ici 2025, l\u2019objectif est de fournir pour la plupart des transfusions des plaquettes et du plasma \u00e0 teneur r\u00e9duite en agents pathog\u00e8nes.   <\/p>\n\n<p class=\"wp-block-paragraph\" id=\"octaplasma\">Octaplasma est le nom de commerce d\u2019une forme de plasma congel\u00e9 \u00e0 teneur r\u00e9duite en pathog\u00e8nes fabriqu\u00e9 par Octapharma. Jusqu\u2019\u00e0 pr\u00e9sent, Octaplasma \u00e9tait r\u00e9serv\u00e9 \u00e0 certaines populations de patients, mais la situation va changer \u00e0 partir du mois prochain, lorsqu\u2019il sera accessible \u00e0 tous les patients. Cet acc\u00e8s g\u00e9n\u00e9ralis\u00e9 est la voie la plus rentable et la plus rapide vers un approvisionnement en plasma r\u00e9duit en agents pathog\u00e8nes.   <\/p>\n\n<p class=\"wp-block-paragraph\">Au moment de la fabrication d\u2019Octaplasma, du plasma venant de centaines de dons est regroup\u00e9. \u00c0 cette \u00e9tape, la quantit\u00e9 d\u2019anticorps qui pourraient \u00eatre pr\u00e9sents dans le bassin de plasma diminue par dilution. Le bassin de plasma est ensuite filtr\u00e9 de fa\u00e7on st\u00e9rile, ce qui en retire les leucocytes, les prions, les parasites et les virus. Le plasma est trait\u00e9 au moyen d\u2019un m\u00e9lange de tri(n-butyl)-phosphate 1 % , ou TNBP, et d\u2019octoxynol 1 %. Ce dernier produit est un d\u00e9tergent qui interrompt les interactions des mol\u00e9cules qui se trouvent dans les membranes lipidiques entourant les virus. La plupart des virus envelopp\u00e9s ne peuvent pas survivre sans leur membrane lipidique, de sorte qu\u2019une exposition \u00e0 ces d\u00e9tergents les d\u00e9truit. Le solvant, TNBP, facilite et acc\u00e9l\u00e8re la r\u00e9action entre la membrane lipidique et le d\u00e9tergent. Le solvant et les d\u00e9tergents sont \u00e9limin\u00e9s par extraction \u00e0 l\u2019huile de ricin (pour le TNBP), puis par extraction en phase solide (pour l\u2019octoxynol). Le plasma passe ensuite \u00e0 travers une colonne de r\u00e9sine pr\u00e9activ\u00e9e pour recevoir un ligand, ce qui extrait sp\u00e9cifiquement les prot\u00e9ines prions. Cette \u00e9tape est consid\u00e9r\u00e9e comme \u00e9tant efficace pour \u00e9liminer l\u2019agent infectieux responsable de la maladie de Creutzfeldt-Jakob (vCJD) s\u2019il est pr\u00e9sent. Le produit final est conditionn\u00e9 dans des sacs de sang standardis\u00e9s de 200 mL et congel\u00e9.          <br\/><br\/>Le traitement par S\/D offre de nombreux avantages : il r\u00e9duit le risque r\u00e9siduel d\u2019infection transfusionnelle qui subsiste apr\u00e8s la s\u00e9lection des donneurs et les analyses des dons; il diminue aussi le risque de transmission pendant la transfusion d\u2019un agent pathog\u00e8ne \u00e9mergent ou inconnu. La litt\u00e9rature sugg\u00e8re en outre que le plasma trait\u00e9 par S\/D peut \u00eatre associ\u00e9 \u00e0 une baisse du risque de r\u00e9actions ind\u00e9sirables, y compris des r\u00e9actions allergiques et le syndrome respiratoire aigu post-transfusionnel (TRALI).<sup>1-5<\/sup> L\u2019utilisation d\u2019Octaplasma est aussi b\u00e9n\u00e9fique pour le syst\u00e8me d\u2019approvisionnement en sang, car elle permet d\u2019augmenter le volume de plasma canadien qui peut servir au fractionnement. <br\/><br\/>Le 27 mars prochain, la proc\u00e9dure de commande de plasma S\/D sera modifi\u00e9e pour permettre aux h\u00f4pitaux de commander Octaplasma pour tous leurs patients. D\u2019ici septembre 2023, tous les h\u00f4pitaux devront commander Octaplasma; au moins 80 % de leurs \u00e9missions de plasma devront \u00eatre constitu\u00e9es d\u2019Octaplasma \u00e0 cette date.
