The Home Infusion Toolkit v 2.0 is designed to provide all hospitals that participate or want to participate in home infusion with an outline of the requirements of such a service and some sample documents to support it. This toolkit does not provide a homogenous home infusion program for the province of Ontario as the scope and structure of these programs varies from hospital to hospital. \u00a0 These sample documents are to be used as a model only in creating your own organizational documents to support your\u00a0hospital\u2019s home infusion program. Those documents provided in WORD can be edited into hospital templates. \u00a0 This toolkit will undergo a formal review in 3 years time. Any changes to the toolkit before that time will be communicated through our \u201cWhat\u2019s New\u201d.
