L\u2019an\u00e9mie ferriprive est le type d\u2019an\u00e9mie le plus courant.<br>For several decades intravenous (IV) iron has been used as a successful treatment modality for iron deficiency and iron deficiency anemia (IDA). It is an option when oral iron is not effective or poorly tolerated or when there is a need to correct anemia rapidly. IV iron is used in various hospital specialty groups including cardiac, oncology, gastroenterology, gynecology and obstetrics. The use of IV iron has expanded in recent years and is now a part of perioperative patient blood management programs.   <br>Auparavant, les formulations de fer intraveineux \u00e9taient associ\u00e9es \u00e0 de graves r\u00e9actions attribuables \u00e0 une lib\u00e9ration rapide du fer. Depuis quelques ann\u00e9es, de nouvelles formulations jug\u00e9es plus s\u00fbres sont apparues sur le march\u00e9. Elles contiennent des noyaux d\u2019hydrate de carbone qui lient plus \u00e9troitement le fer \u00e9l\u00e9mentaire, ce qui permet une lib\u00e9ration plus lente et mieux contr\u00f4l\u00e9e tout en r\u00e9duisant les r\u00e9actions<sup>2<\/sup>. Une de ces nouvelles formulations est le d\u00e9risomaltose ferrique (Monoferric\u00ae), auparavant connu sous le nom d\u2019isomaltoside de fer 1000. C\u2019est un compos\u00e9 de fer i.v. de troisi\u00e8me g\u00e9n\u00e9ration qui permet une perfusion \u00e0 forte dose, allant jusqu\u2019\u00e0 20 mg\/kg ou un maximum de 1500 mg administr\u00e9 en 30 minutes ou plus en une seule visite comparativement \u00e0 plusieurs visites. D\u2019abord lanc\u00e9 en Europe en 2009, il a \u00e9t\u00e9 approuv\u00e9 par Sant\u00e9 Canada en 2018. Les essais PROPOSE (N=351), FERWON-Nephro (N=1538), PROVIDE (N=511) et FERWON-IDA (N=1512) ont compar\u00e9 l\u2019isomaltoside de fer 1000 au fer sucrose dans des cas d\u2019an\u00e9mie ferriprive. Tous ces essais ont d\u00e9montr\u00e9 que l\u2019isomaltoside de fer 1000 n\u2019\u00e9tait pas inf\u00e9rieur au fer sucrose pour augmenter ou maintenir le taux d\u2019h\u00e9moglobine4,5,6.  <sup>,4,5,6<\/sup><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><br>La dose totale recommand\u00e9e se situe g\u00e9n\u00e9ralement entre 1000 mg et 1500 mg. La dose peut se calculer \u00e0 l\u2019aide de la formule de Ganzoni ou en consultant le tableau simplifi\u00e9 qui se trouve dans la monographie du produit. Le d\u00e9risomaltose ferrique co\u00fbte 45 $ per mL, donc 450 $ pour un traitement de 1000 mg, ce qui se compare \u00e0 un traitement de trois doses de Venofer, un fer sucrose. Voici une liste de diverses possibilit\u00e9s de couverture du prix du m\u00e9dicament en Ontario.   <\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Paiement par le patient<\/li>\n\n\n\n<li>Assurance priv\u00e9e &#8211; DIN #02477777<\/li>\n\n\n\n<li>Prestation de m\u00e9dicament \u00e0 usage limit\u00e9 (LU) couvert par le programme Programme des m\u00e9dicaments de l\u2019Ontario. Il permet de combler tous les besoins ou presque en une seule perfusion et il est habituellement bien tol\u00e9r\u00e9.
